If you’re having trouble using the online request form or would prefer to submit your request via Email, then please click on the blue Word doc link immediately below. A dialog box will open – be sure to select ‘Save File’ – then click ‘OK’ and the file will be saved to your computer:
That should do the trick! (If you’re still having trouble, you can get the request form by using the contact form at the bottom of this page and someone will get back to you. This will take a while longer since the response to you is not automated.)
We suggest you save the blank document first, then fill it out and also save a completed copy for yourself, then email it directly to us at email@example.com

If you’re coordinating with other schools in your area, then please let them know that you have submitted your request and that they should get theirs in right away as well! We won’t be able to seriously consider dates until we hear from them!
